Permission Slip Music Department Trip to What: The Middle School Chorus, Orchestra, and Band will be traveling to When: Friday, May 8, 2009 DEPART from H-B at RETURN to H-B between Cost: TBA (covering performance fees, awards ceremony, buses, and park admission) Other: 1. All field trips are considered school activities. All school rules and county rules apply while students are on the buses, in the park, and at the performances. Students are expected to consider this a school function. 2. While in the park, students will be expected to have a buddy at all times. There will also be chaperones stationed at a designated point throughout the day in case of problems. Students will be able to check in at these checkpoints. 3. Below you will find how much your son or daughter still owes based on his or her fundraising to date. Additional costs have been added on to student accounts for District fees, auditions, etc. 4. The balance is due by Friday, March 6th along with this permission form. 5. If you have any questions, feel free to call the music department at 703-228-2215 or email Bonny, Carl or Risa.
